Service Description

Ages 14–22+ Preparing for adulthood involves more than transition goals in an IEP. Many families are surprised by how complex the shift from school-based services to eligibility-based adult systems can feel. This service provides structured, consultative guidance to help families understand commonly available Massachusetts programs and prepare application materials with clarity and organization. Areas of Focus: • Understanding the shift from entitlement to eligibility-based systems • Exploring programs such as DDS, SSI, MassHealth/CommonHealth, PCA, and vocational rehabilitation • Timeline planning for key milestones (ages 14–22+) • Customized documentation checklists to streamline preparation • Guided review of completed applications prior to submission Services are educational and consultative in nature. Final submission of applications and eligibility determinations remain the responsibility of the family and respective agencies.
